Why We Use Real Ear Measurement at Every Fitting

Many people expect getting fitted for hearing aids to work the same way as getting glasses. The audiologist looks at your test results, quickly adjusts the settings, and you walk out ready to go. It’s an understandable assumption, but hearing is more complicated than that. It’s also one reason some people end up with hearing aids that never feel quite right.

Real Ear Measurement is what makes the difference between devices that really work for you and ones you might leave on your dresser unused. We perform REMs at every fitting here at Audiologic Solutions, and the difference it makes surprises most patients.

No Two Ear Canals Are the Same

When a hearing aid is first programmed, the software is working from averages. Your audiogram, the results of your hearing test, tells it how much amplification to deliver at each pitch. It is a solid foundation, but still just a starting point.

What those averages can't account for is the shape, length, and volume of your own ear canal. Sound behaves differently depending on the space it travels through. Two patients with matching audiograms wearing the same model can have completely different experiences with it. One hears clearly, while the other finds the sound sharp, loud, or just vaguely off.

That's the problem real ear verification exists to solve.

What Happens During the Measurement

The process is simple, and there's nothing uncomfortable about it. We slide a thin, flexible probe tube, roughly the width of a strand of spaghetti, into your ear canal alongside the hearing aid. Then we play sound through a speaker and measure what's actually reaching your eardrum in real time.

Those measurements get compared against a prescriptive target built from your specific hearing loss. Too much amplification in the high frequencies shows up right away. Under-amplified soft speech sounds, the ones that carry so much of a conversation, show up too. We adjust until what the device delivers lines up with what your ears need. Verification instead of guesswork.

Why Factory Settings Fall Short

First-fit algorithms have come a long way, and manufacturers put real work into them. Research on fitting accuracy keeps landing in the same place, though. Default settings miss prescriptive targets in a large share of cases when nobody verifies them afterward.

A patient can walk out with a device that isn't matched to their hearing loss and never suspect a thing. Acceptable and correct aren't the same standard.

Your brain deserves some of the blame for that. After years of gradual hearing loss it gets very good at filling in gaps on its own, so you can't judge a fitting by asking yourself whether it sounds fine. Fine is a low bar.

What Changes With a Verified Fitting

At first the difference can be subtle. Over a few weeks, it usually isn't.

Patients fitted with real ear verification report clearer speech understanding in restaurants and other noisy rooms, less mental fatigue by the end of the day, and fewer complaints about harsh or grating sound. That last one carries more weight than people expect. Over-amplification in the high frequencies is one of the most common outcomes of skipping verification, and it's a leading reason hearing aids end up in a drawer. Patients tell us things sound tinny, or that background noise has become unbearable.

Most of the time that's a measurement problem, not a hearing aid problem. A good fitting leaves you hearing more clearly, not simply hearing more.

Every Fitting, Not Just the First One

Hearing shifts. Ear canals change shape gradually with age. Components drift in performance over the life of a device. A new hearing aid from a different manufacturer can respond differently from the one you wore before.

None of that turns up on an audiogram. It turns up in the measurement, which is why we verify at follow-up appointments and not only at the initial fitting. Yes, it adds time to your visit. We think that's an easy trade.
